Bilirubin levels are commonly tested in ascitic fluid in patients suspected of bile duct leak or gall bladder perforation. High level supports the diagnosis of Choleperitoneum. A study has shown that drain fluid to serum bilirubin ratio of >5 is indicative of bile leak. Ascitic fluid to serum bilirubin ratio of >3.25 is highly specific for bile leak in liver transplant recipients. In other fluids like pleural fluid, this test is used to differentiate between transudates & exudates. |
