Specimen: 2 mL (0.5 mL min.) serum from 1 SST for each timed specimen. Draw fasting early morning baseline sample between 8-10 am; administer 250 ug of synthetic ACTH (Synacthen) IV, draw additional sample at 60 min. Ship refrigerated or frozen. Both tubes must be clearly marked with time drawn. Submit both tubes with one test request form.

Usage: Measuring the response of 17-Hydroxy progesterone (17-OHP) to a 60-minute challenge with intravenous ACTH is the single most powerful & reliable means to diagnose 21-hydroxylase deficiency. Women with Non-classical Congenital Hyperplasia (NC-CAH) due to 21-hydroxylase deficiency and women with Polycystic Ovarian Syndrome (PCOS) have a similar clinical presentation, with hyperandrogenism, oligomenorrhea, and polycystic ovaries. As the line of treatment may differ in the two conditions, it is recommended to differentiate NC-CAH from PCOS. This test helps to distinguish NC-CAH from PCOS.
